Requirements for Athletic Participation

Please read the following requirements carefully.

If the requirements are not met, you will not be permitted to practice or compete until all information is received and all steps are completed.

**If you are late for your assigned time, you will not be permitted to enter. If you miss your time, you will not be permitted to participate in any team activities until the testing is completed. You will not be rescheduled until all other tests are completed. **

Deadline for all forms to be completed and mailed in is JULY 16, 2021.

Each athlete is required to have a yearly physical examination and athletic screening prior to any participation in Saint Vincent intercollegiate sports.
Physicals are performed by the athlete’s primary care physician prior to mailing in the completed forms and paperwork.
Athlete must complete and/or update information at www.swol123.net (tutorial video can be found above).
Athletes must have proof of current health insurance coverage in order to participate in intercollegiate sports at Saint Vincent College.
Completed forms (Forms A, B, C, D, E, F, G, H, I, J, K and L) along with a copy of front and back of current health insurance card must be mailed to your coach by July 16, 2019. Two sport athletes, please mail information to the coach for which sport you will practice in first.
Athlete Insurance and Emergency Information Form, Form A must be typed on the computer and then printed and signed by athlete and parent prior to returning to assure accurate information.
If you have a diagnosis of ADD or ADHD, complete the Medical Exception Form for ADD/ADHD found on www.swol123.net.
Athlete with a recent surgery, illness, or injury and was/is under the care of a medical specialist, please include a written release to participate in athletics from that specialist.
See chart below for date, time and location of specific athletic screenings.
Athletes WILL NOT be allowed to participate in any athletic competition or practice until their athletic screenings are complete and all information is on file in the athletic training office.
The final decision on physical qualification or reasons for rejection is the responsibility of the Saint Vincent College team physician(s) and athletic trainers.
The team physician(s) and/or athletic training staff also make the decision on when an athlete may return to competition after an injury.

After you have completed the Pre-Participation Athletic Physical Examination with your physician, mail ALL completed and signed forms (Forms A, B, C, D, E, F, G, H, I, J, K and L) along with the copy of the front and back of your current health insurance card and any other necessary documentation to YOUR COACH at:
